Health Insurance Policies

As a courtesy, Associates in Rehabilitation Medicine will file your insurance claim for you. Therefore, at registration, you will be asked to present your health insurance card and sign a form assigning insurance benefits to us. Please review your policy for managed care plan requirements. Together, we will ensure that all pre-approval requirements are met.

If applicable, you will need to present the appropriate referral form from your primary care physician. If you do not have your referral form at the time of your visit, we will attempt to receive it by fax from your primary care physician. If a referral is not available, you will be given the choice to self-pay or reschedule your visit. If your health insurance plan fails to make payment within 45 days from the date of visit, you will be asked to make other payment arrangements.

Managed Care

Associates in Rehabilitation Medicine has entered into contract with various managed care organizations, including Point-of-Service (POS) programs, Preferred Provider Organizations (PPO) and some Health Maintenance Organizations (HMO). It is your responsibility to verify that Associates In Rehabilitation Medicine is a participating provider in your managed care plan.

Payment

You will be responsible for the bill if the service is not a covered benefit; the service is not deemed medically necessary by your insurance company; and your managed care plan requires you to pay deductibles, co-payments and/or co-insurance.

For all patients who must personally pay all or part of their healthcare bills, we accept check, MasterCard, Visa or cash. Upon receipt of our billing notice showing your balance due, you are expected to make payment in full or contact us to make other arrangements.

Third-Party Liability

For patients treated for injuries from an automobile or work injury, we will attempt to receive authorization from the appropriate carriers before your visit. If prior authorization is not received, you will be responsible to self-pay for the visit. As a courtesy, we will forward an itemized bill to your attorney upon written request from the attorney.
